Advantage/Disadvantage to Rolls - Should not be applied in encounter

Franz is applying advantage to all his attack rolls, while applying disadvantage to all player attack rolls for no reason. No abilities, spells, items, or equipment has been used to cause this. I provide feedback to Franz directly, he states he has corrected this error and can move forward with process normally, but still continues applying advantage/disadvantage to rolls.
