Missing items and Not remembering what happened earlier

I am missing several items I obtained early on in the campaign. The AI does not remember the wizard that gave me an item to telepathically communicate with him and the item is no longer in my possession along with numerous other things such as weapons, food, drink, supplies, armor. This is very immersion breaking.
